Medical Cannabis and Your Documents

First, create a Your Choice Medicinal account by selecting a username and password.  Then, click the "Get Started" button and complete the Medical Document Request form.  You will be provided with the option of speaking to a Health Care Provider about your medical cannabis needs.  After submitting the form, a Health Care Provider will review your submission to determine whether this treatment option is appropriate for you based on Health Canada guidelines. 

A medical cannabis document is like a prescription, but it’s not one. It is a form completed by a doctor or nurse practitioner that authorizes you to purchase cannabis products from Your Choice Medicinal. We offer this service free of charge, however, there is a $30 fee associated with rescheduling a missed doctor’s appointment.

Medical cannabis is prescribed by a healthcare practitioner to treat specific health conditions. It is accessed through licensed medical providers, may be tax-deductible, and can be eligible for insurance coverage. You can get access to personalized treatments, dosing information, and medical documentation.

Recreational cannabis is for personal use without a prescription. It’s sold through retail stores or online at the Ontario Cannabis Store. It is not covered by insurance and is not tax deductible. No medical oversight is provided.

Accessing or Transferring Your Medical Document

Ask your original clinic to email your medical document directly to Your Choice Medicinal, at [email protected].

Once your transferred document has been processed and recorded, you will receive an email with a link to get started. This typically takes 1-5 business days.

Please note, if your medical document will expire within the next 30 days or less, you must get a new medical document. You can get a new medical document through the YCM clinical team by clicking HERE 

In most cases, you do not need to inform your insurer about changing your medical cannabis provider. However, since each insurance policy is different, it’s best to contact your insurance provider to confirm whether you need to inform them about changing medical cannabis providers.

After submitting your request for a medical cannabis document, or after your appointment, the YCM physician will review your case to determine your eligibility.  Typically, your medical document will be completed and submitted within 1-3 business days.  

Once your medical document has been processed and recorded, you will receive an email with a link to get started. This typically takes 1-3 business days.

You may only get your medical cannabis directly from your licensed medical cannabis provider. Medical cannabis providers are not permitted to have retail locations. If you go into a cannabis store in your neighbourhood, you will be purchasing cannabis outside of your medical allotment. Please note that any recreational cannabis you purchase is not eligible for coverage by your insurer.

Appointment Scheduling & Info

Given the number of patients the YCM doctors attend to, it is best to discuss your ongoing medical cannabis needs with your GP/family doctor or another member of your health care team.

After booking your appointment, you will get 2 appointment reminders: one immediately after you book, and one on the day before your appointment.

Please contact [email protected] if you need to cancel or reschedule your appointment.  You may cancel or reschedule up to 24 hours before the appointment.  

Late cancellations or missed appointments will not result in a fee, however, in these cases, a fee of $30 will apply in order to reschedule the appointment. 

Your Order

All orders placed before noon will ship same day. Orders placed after noon will ship the following business day. Shipping times range from 1-3 business days. After placing your order, you will receive an email with your order details, an official receipt that you can submit to your insurance provider and a Canada Post tracking number. Shipping

Shipping times vary based on your distance from the YCM shipping location.  Shipping times range from 1-5 business days.  All orders include a tracking number for your reference

Shipping typically costs $20 per order and is free for orders that are $120 or more (before HST).
